- Definitions: hazard, accident, near miss;
- Accident Causation Models: domino theory and multi-causality;
- Immediate and underlying causes, including management system failures and human factors
- Accident Investigation Protocol – what should be investigated and by whom;
- Accident Investigation Procedure:
- Immediate action following accident;
- Level of investigation and persons involved;
- Gathering detailed information including photographs and witness statements;
- Analysing the information;
- Identifying suitable risk control measures;
- Action plan and implementation.
- Practical exercise on accident investigation
- End of Course Assessment.
Any person involved in the accident investigation process, particularly supervisors, managers, health & safety professionals and employee representatives.

Location: The course is normally held on client's premises, and delivered specifically for their employees. If required, the course can be held on Holland HSC's premises (at extra cost).
